What is an asthma action plan and why is it important?

Action plans reduce hospital visits, improve symptom control, and ensure consistent management. They're especially important for children and people with moderate or severe asthma. How do I know if my asthma is well controlled?
Asthma is considered well controlled if symptoms occur no more than two days a week, you rarely wake at night due to asthma, and you don’t need to use your reliever more than twice a week. You should also be able to carry out daily activities without limitations.
